Oh no, we're so sorry your phone has cracked! Please fill out this form completely ONCE YOU HAVE REPAIRED YOUR PHONE so we can get the ball rolling on your reimbursement. :)

PLEASE NOTE:

  • Payments are made to PayPal ONLY, do not include your PayPal link, only your PayPal email address.
  • Payments are made every Wednesday morning. If your information is received Wednesday afternoon, it will be processed the following Wednesday.
  • Please include 1-2 photos of your cracked screen with your Pela Case on your phone. 
  • Please include a photo or PDF of your repair receipt.
  • Please include both order numbers if Canopy and your case were purchased separately.
